PURPOSE:

The primary purpose of this position is to provide transportation for residents at the Rouse Home and Suites at Rouse for appointments and personal outings, to drive, and/or accompany residents to medical appointments, complete errands, run labs to the hospital, assist with clerical duties, and assist with 360 Care services as needed to help the Transportation Coordinator.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Ensure that resident’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage a complaint are followed by all department personnel.
  2. Maintain CONFIDENTIALITY of all pertinent resident care information to assure resident’s rights are protected.
  3. Report any changes in resident’s physical or behavioral conditions, while out of the facility to charge nurse.
  4. Complete resident shopping for personal items as requested, management of resident funds/receipts.
  5. Drive/transport/accompany residents as necessary to medical appointments. Operate wheelchair lift safely and assist resident with feeding, toileting, and other CNA-related duties while resident is under transportation aide’s care.
  6. Coordinate errands (i.e. bank, courthouse, lab , etc.) with other transports to achieve maximum efficiency; as required; keep other departments informed as to the next available transport.
  7. Assume the authority, responsibility and accountability of your position.

  1. Direct visitors to resident’s rooms, office areas, etc. as necessary.
  2. Develop and maintain a good working rapport with interdepartmental personnel, as well as other departments’ services and activities can be properly maintained to meet the needs of the residents.
  3. Perform related work as required by Department Director and Transportation Coordinator.
  4. Report all hazardous conditions and equipment to the department director or maintenance department immediately.
  5. Maintain hazard free work area.
  6. Comply with local traffic regulations.
  7. Inspect vehicles before departure and report issues to Maintenance and Transportation Coordinator.
  8. Assist Life Enrichment with 1:1 social contact visits and group activities.
  9. Deliver weekly transportation schedules to households and update any “last minute” changes at the direction of the Transportation Coordinator.
  10. Update and coordinate with POA/Responsible Party for medical and social outings.
  11. Prepare transportation folders with medical orders, POLST, Insurance Cards, Therapy Notes, Face sheet, and Route Slip (or any other pertinent information for the appointment).
  12. Prepare Route Slips, enter notes in Point Click Care, add the transportation details into Life Loop, and assign a driver for each trip.
  13. Keep vehicles clean, sanitized, filled with supplies, and maintain fuel levels.
  14. Upload route slips and other necessary documents into resident charts.
  15. Coordinate personal outings at the resident/POA/Responsible Party request.

 

WORKING CONDITIONS:

  1. Driving, lifting, sitting, standing, bending and moving intermittently during working hours.
  2. Willingness to work beyond normal working hours when necessary.
  3. Attend and participate in continuing educational programs.
  4. Communicates with the medical staff, nursing personnel and other departments.

 

QUALIFICATIONS:

 

Education:

  1. Must possess a high school diploma or equivalent.
  2. Must have and be willing to maintain current CNA certification.
  3. Must have valid and current PA driver’s license and good driving record.
  4. Must be willing to obtain and maintain CPR certification.

 

Experience:

Knowledge in resident care, basic clerical skills and basic care of vehicles is desired.

 

Special Requirements:

  1. Must follow all Rouse safety and transportation policies and be able and willing to successfully complete training in operation of Rouse vehicles.
  2. Possess the ability to deal tactfully with personnel, residents and the general public.
  3. Possess the ability to make independent decisions when circumstances warrant such actions.
  4. Ability to work harmoniously with other personnel.
  5. Ability to resolve problems that arise such as mistakes, costly waste of supplies, misuse of equipment, settling personnel grievances, etc.
  6. Possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.

 

Physical Requirements:

 

  1. Must be able to move intermittently throughout the day, i.e.: transport wheelchairs safely, assist residents, operate wheelchair lift, climb in/out of mini bus several times daily, lift and carry medical equipment.
  2. Must be able to independently lift up to 50 pounds regularly.
