Your Role: Empowering Patients Through Compassionate Care Management
As an RN Care Manager, you'll play a vital role in ensuring patients receive the support and resources they need. You'll work closely with patients, families, medical professionals, and our team to develop and implement personalized care plans. This role offers the perfect blend of autonomy and collaboration, allowing you to make a meaningful impact while maintaining a healthy work-life balance.
Your Responsibilities:
Patient Advocacy: Champion your patients' needs, ensuring they receive appropriate services based on their choices and program eligibility.
Care Planning: Develop comprehensive care plans based on thorough patient assessments.
Communication & Collaboration: Effectively communicate with patients, families, medical professionals, and our internal team, both verbally and in writing.
Documentation: Maintain accurate, complete, and timely patient documentation.
Emotional Support: Provide social and emotional support, offering problem-solving techniques to patients and their caregivers.
Coordination & Monitoring: Ensure seamless coordination of care and monitor patient progress.
What We're Looking For:
Graduate of an accredited School of Nursing.
Current RN license in the State of Michigan.
Minimum of one year of experience as a Registered Nurse, with at least one year in Home Health Care.
Case Management certification is a plus!
Strong knowledge of primary care practices and regulations.
Excellent interpersonal and communication skills.
Ability to perform physical tasks, including lifting and standing, as needed.
Reliable transportation and the ability to travel within the service area.
Ability to work in a hybrid environment.
Residency within the state of Michigan.
