Job Description

Position: Community Partner Care Coordinator
Location: Remote (Must Reside in Tennessee)
Schedule: Full-Time, Monday - Friday, 8-hour shifts during normal business hours (no weekends!)

About the Role:
We're seeking a dedicated Community Partner Care Coordinator to support Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan (DSNP) members. In this role, you will build meaningful relationships with members, assisting them in navigating healthcare services, accessing resources, and addressing both medical and social needs. By advocating for members and coordinating care, you'll help improve their health outcomes and overall quality of life.

Key Responsibilities:
Engage and Educate Members: Conduct outreach via phone and text to motivate members and explain program benefits.
Assess and Develop Care Plans: Evaluate members' needs and collaborate with them to create personalized care plans, ensuring their health and social goals are met.
Coordinate Care: Schedule and facilitate appointments with healthcare providers, ensuring timely and appropriate care.
Address Social Determinants of Health (SDOH): Identify and connect members with community resources to address non-medical factors that impact health.
Document Member Interactions: Accurately record case notes and care coordination activities in an electronic health record or care management platform.
Maintain High Member Engagement: Manage a caseload of members, ensuring consistent contact and follow-up to keep them actively involved in their healthcare.
Foster Collaborative Relationships: Work closely with members, their families, healthcare providers, and community organizations to promote positive health outcomes.
What We're Looking For:
Required Qualifications:

2+ years of experience as a care coordinator, case manager, community health worker, pharmacy technician, or social worker.
Familiarity with Medicaid and/or Dual Eligible patient populations.
Fluency in English (Spanish fluency is a plus).
Ability to work remotely from a dedicated home office.
Reside in Tennessee.
Preferred Qualifications:

LPN or RN credentials.
Experience with motivational interviewing techniques.
Proficiency with Apple computers (MacBook experience preferred).
Previous experience working in care management or electronic health record systems.
Skills and Attributes:
