Are you passionate about making a meaningful difference in people's lives? As a Care Coordinator, you will play a key role in supporting Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan (DSNP) members, ensuring they receive the care they need when they need it. You'll work closely with members to build trust, guide them through the healthcare system, and connect them with the right services. This role allows you to be an advocate for members, addressing both their medical needs and social factors that affect their well-being.
What You'll Do:
Build Relationships: Establish trusted connections with members, offering guidance and support to help them navigate their healthcare journey.
Coordinate Care: Work with healthcare providers, pharmacies, community organizations, and other partners to ensure members receive comprehensive care.
Assess and Plan: Conduct assessments to understand each member's health status and create personalized care plans.
Address Social Needs: Recognize and address Social Determinants of Health (SDOH) to support overall member well-being.
Communication: Engage with members via phone and text to educate, motivate, and assist with program benefits, appointments, and more.
Document Actions: Maintain accurate records in the care management platform, ensuring all actions are well-documented and aligned with best practices.
Who We're Looking For:
Experience: 2+ years in a care coordination, case management, community health, or similar role (pharmacy tech, social worker, etc.).
Skills: Strong communication (both verbal and written) and problem-solving abilities. Comfortable working with clients over the phone and through text.
Technical Proficiency: Experience using Apple computers (e.g., MacBook) and documenting in care management or electronic health records platforms.
Motivational Skills: Familiarity with motivational interviewing techniques to empower members to take control of their health.
Language: Fluency in English required, with Spanish fluency highly preferred.
Location: Must reside in Tennessee.
License/Certification: LPN or RN preferred.
