Job Description
We are looking for a dedicated and detail-oriented Clinical Pharmacist to join our team! In this role, you will play a key part in ensuring the safe and effective distribution of medications across all patient care areas. You'll have direct responsibility for medication distribution, IV admixtures, and monitoring prescribed drug therapies to ensure compliance with state and federal regulations. Additionally, you'll serve as a trusted resource for healthcare providers and patients, offering valuable drug information and recommendations.
Key Responsibilities
Conduct thorough clinical reviews of medication orders to ensure safety and efficacy, considering factors such as allergies, dosage, interactions, and patient-specific parameters.
Verify and oversee the preparation of sterile and non-sterile medications.
Ensure compliance with all state and federal pharmacy regulations.
Provide drug information and guidance to medical staff, nurses, and patients.
Document and communicate medication-related details with the healthcare team to maintain continuity of care.
Supervise pharmacy students, technicians, and residents, fostering a collaborative and educational work environment.
Actively contribute to medication safety initiatives, including reporting medication-related events and near misses.
Participate in daily pharmacy surveillance monitoring and support patient care initiatives.
Engage in continuous quality improvement efforts, offering suggestions for optimizing workflow and operations.
Support the development of clinical guidelines, policies, and educational programs for the hospital.
Serve on interdisciplinary hospital committees or workgroups to enhance patient care processes.
Assist with research activities, medication use evaluations, and cost-saving initiatives.
Uphold a culture of teamwork, respect, and professional excellence within the pharmacy department.
Qualifications
Graduate of an accredited college of pharmacy with an active Massachusetts pharmacist license (or eligibility to obtain one).
Advanced certification (e.g., BPS) in a relevant specialty within two years of hire.
Strong knowledge of health-system pharmacy practices, including drug therapeutics and sterile compounding.
Relevant experience, including completion of PGY-1 plus 1-3 years of experience, or PGY-1 plus PGY-2 residency (or equivalent training).
Excellent communication and problem-solving skills with a patient-centered approach to care.
