Job Description

We are seeking someone to be an integral part of a multi-year growth plan to treat patients with skull base conditions from across the Midwest. The chosen candidate will work alongside skull base ENTs, cranial neurosurgeons, endocrinologists, radiation oncologists, head/neck cancer surgeons who are nationally recognized in the treatment of acoustic neuromas and CSF leaks.

The Neuroscience/Skull Base Nurse Navigator is responsible for the seamless coordination of care across the continuum for patients with conditions that are complex and often require multiple visits and a combination of treatment methods.

The nurse navigator is a patient advocate who:
· Coordinates care in conjunction with clinical team from time of diagnosis through discharge
· Educates patient and family/significant others on the health care experience
· Implements, coordinates, monitors, and evaluates care options, services, and resources.
· Links family/patient to appropriate resources as needed.
· Proactive in finding ways to enhance the patient experience clinically and in service.

Additionally, the nurse navigator will:
· Assist in quality improvement initiatives by gathering, tracking, and researching quality metrics.
· Monitor outcomes, e.g. patient experience & satisfaction, time from referral to physician evaluation, communication with referring providers
· Collaborate with physicians, nurses, case managers, and others to ensure appropriate tests and treatments are delivered in a timely fashion.
· Perform other duties to expand Skull Base Program

Requires an Associates of Nursing (ASN).

Salary: $ 31.00 43.00 Per Hour
