Position Overview
This fully remote position supports patients and medical practices in the Atlanta, GA area. Ideal candidates will be based in the Eastern or Central time zones.
As a Patient Care Coordinator, you'll play a crucial role in strengthening the connection between patients and their primary care providers, enhancing patients' health and well-being. You'll engage with patients to help them navigate their care, address healthcare issues, and improve communication with their medical practice.
Key Responsibilities
Collaborate remotely with patients' medical practice staff and our Case Management team to manage a targeted patient population.
Work independently and as part of a team to assist patients in achieving better health outcomes.
Address patient questions and concerns, connect them with medical practice staff, implement care plans, and close care gaps.
Navigate patients to appropriate resources, including referrals to specialty care and community-based organizations (CBOs).
Accurately document patients' medical history and issues in the electronic health record (EHR).
Understand and manage the referral and prior authorization processes.
Build and maintain trusting, professional relationships with medical practices and our organization.
Meet assigned Key Performance Indicators to ensure positive patient outcomes and reduce medical costs.
Use effective communication methods (calls, texts, portals, emails) to engage high-risk patients.
Escalate medical issues to the Health Guide Registered Nurse, Primary Care Physician, or leadership as needed.
Occasionally travel to primary care physician offices.
Follow all security policies and procedures to protect patient information and organizational assets.
Qualifications
Bilingual in Korean and English; Korean/English medical interpreting certification preferred.
Passion for patient coordination and serving individuals with complex medical and social needs.
Empathy and proficiency in motivational interviewing, critical listening, and behavioral change management.
Exceptional communication skills and the ability to document patient interactions accurately and in real time.
Experience with medical terminology and primary care physician practices.
Knowledge of social determinants of health.
Bilingual in English and Spanish is a plus.
Certified or Registered Medical Assistant or Certified Emergency Medical Technician preferred.
Comfortable using electronic health records (EHRs), care management databases, and various software programs.
Ability to manage multiple priorities while maintaining attention to detail.
Familiarity with EHR systems such as EPIC, Athena, or Cerner.
Strong written, visual, and verbal communication skills.
Proven ability to work independently in a remote environment.

Compensation
The base pay for this role ranges from $20 to $23 per hour, paid bi-weekly. Final compensation will depend on factors such as experience, education, location, and certifications.
