Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care across diverse healthcare settings. The ideal candidate will possess strong clinical skills, a thorough understanding of medical terminology, and a commitment to patient-centered care. As an LPN, you will play a vital role in supporting physicians and interdisciplinary teams by performing assessments, administering treatments, and ensuring the comfort and safety of patients. This position offers an opportunity to work in dynamic environments such as long-term care facilities, hospitals, outpatient clinics, or home health settings.

Responsibilities

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic or Cerner.
  • Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA regulations and infection control standards.
  • Assist with invasive procedures such as IV infusion, catheterization, venipuncture, and sterile dressing changes using aseptic techniques.
  • Monitor patients’ conditions closely, recognizing signs of acute pain or deterioration requiring immediate intervention or triage.
  • Collaborate with physicians and healthcare teams to develop and implement individualized care plans for patients with complex needs such as dementia, Alzheimer's disease, or developmental disabilities.
  • Provide specialized care for diverse populations including pediatric, neonatal, geriatric, hospice, or trauma patients in settings like Level I Trauma Centers or NICUs (Neonatal Intensive Care Units).
  • Educate patients and their families on health management strategies including nutrition, medication administration, wound care, and discharge planning.
  • Maintain accurate medical documentation and ensure compliance with all regulatory standards including ICD coding for diagnoses such as ICD-9/ICD-10.

Requirements

  • Valid licensure as a Licensed Practical Nurse (LPN) from an accredited state board of nursing.
  • Proven experience in critical care environments such as ICU or emergency medicine is highly desirable.
  • Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for documentation and patient management.
  • Experience working with diverse patient populations including those with dementia, memory care needs, or behavioral health conditions.
  • Knowledge of advanced nursing procedures including ventilator management, tube feeding, sterile processing, and post-acute care protocols.
  • Strong understanding of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
  • Ability to perform phlebotomy, administer injections safely, and manage complex medication regimens including pain management strategies.
  • Excellent communication skills combined with a compassionate approach to patient care in fast-paced environments such as hospital medicine units or outpatient clinics.
