RN Team Leader - Hospice Manhattan
HOURS: M-F 9-5, no on call, no weekends
DUTIES:
Supports the Hospice Nurse Manager position by providing leadership guidance to the professional staff on a day-to-day basis to ensure the provision of Hospice and Palliative Care services are consistent with the philosophy, goals, and policies of the program and MJHS. Mentors and provides leadership direction to the professional staff as directed by the Hospice Nurse Manager.
50% in their Manhattan office, 50% in the field communicating with their Hospice Field Case Managers from the office and making sure everything is going smoothly, troubleshooting, being the 1st line of response to field staff regarding issues or patient care related questions, 50% in the field will be actually having a caseload of 7-9 patients, occasionally performing sup visits
QUALIFICATION: RN, BSN, strong Hospice a must (5+ years), Supervisory exp preferred but not a must, they will consider a strong Hospice Field CM looking for a step up
